2025 Community Service Award Nomination Form

The Table Rock Lake Chamber of Commerce Community Service Award will be presented to an individual or an organization who, through unusual services and commitment, has improved one or more of the following characteristics of the community: quality of life, economy, educational opportunities, beauty, serenity and safety. Any person or organization is eligible to receive the award, except that award cannot be made to a member of the current Board of Directors or an employee of the chamber or an elected or appointed local, state, or governmental official.

Instructions to Nominate

Please complete entire form as incomplete submissions cannot be accepted. Voting will take place solely on the basis of this form. Note that you must be employed by a Chamber member to submit a nomination, and self-nominations will not be accepted from the business owner or staff. The deadline to submit nominations is 5 p.m., September 5, 2025. Be sure to be specific on why you think your nominee is deserving.
