2025 Trailblazer Award Nomination Form

The Table Rock Lake Chamber of Commerce will present this award to an individual in the Table Rock Lake area that has forged the path for others in our community. This individual has demonstrated invaluable leadership either in business or community development in the past (at least 10 years ago), and influenced the community in a positive manner.

Instructions to Nominate

Please complete entire form as incomplete submissions cannot be accepted. Voting will take place solely on the basis of this form. Note that you must be employed by a Chamber member to submit a nomination and self-nominations will not be accepted from the business owner or staff. The deadline to submit nominations is 5 p.m., September 5, 2025. Be sure to be specific on why you think your nominee is deserving.
