2025 Guardian Award Nomination Form

The Table Rock Lake Chamber of Commerce will present this award to a first responder (law enforcement, firefighter, paramedic/EMT or 911 dispatcher) to recognize the fearless and unselfish service in helping keep our community safe.

Instructions to Nominate

Please complete entire form as incomplete submissions cannot be accepted. Voting will take place solely on the basis of this form. Note that you must be employed by a Chamber member to submit a nomination and self-nominations will not be accepted from the business owner or staff. The deadline to submit nominations is 5 p.m., September 5, 2025. Be sure to be specific on why you think your nominee is deserving.
